Hello and welcome to IWU Titan family!
On behalf of our Sports Medicine Staff, we would like to welcome you to Illinois Wesleyan. As a part of becoming a Titan, our medical staff requires you to complete some online documentation regarding your health history, insurance and contact information. Please note these forms are in addition to those required by Arnold Health Services as a new student so be sure to follow their requirements as well.
The following link will take you directly to our online documentation, login screen (https://www.swol123.net/default.aspx). Once you either click on the link or copy and paste it into your browser, it should allow you to create an account/JoinSportsware. (The school ID is IWUTITANS.) We will receive notification of your account access and then accept you into our system. We are usually fairly quick on that but it could take up to 2 business days.
Once accepted, you will then receive an email (from the Sportsware system) which will allow you to access all of our preliminary documentation. After you create an account, you MUST GO BACK INTO Sportsware and fill out the proper information after you are accepted. The link to finish creating your account only lasts for 24 hours so if the link no longer works, go to the Sportsware home page and type in your email and reset password.
Please use the attached step-by-step guide to complete all of the required fields. You need to complete ALL of the information that is applicable to you in each of those tabs as well as the attached documents. This information includes contact information, emergency contact information, insurance information, medical history questionnaire, HIPAA/Consent to treat form, drug testing consent form, and CCIW acknowledgement form.
NCAA Division III requires each student-athlete to submit proof of sickle cell status to the sports medicine staff in order to participate. You may not sign a waiver to decline testing, submitting results are mandatory. Below you will find some links with information on how to obtain sickle cell status, this may take some time to receive so don't wait until the last minute on this.

If at any point you have any questions, please feel free to contact me (through email or my office number which is listed below) and I will do my best to help you. In addition, we MUST HAVE a copy of your physical and insurance card as well as your sickle cell status on file before you can begin practice/workouts with the team. We will accept any physical completed by a physician within the last 6 months prior to the start of the 2025-26 school year. Our specific physical form can be found as an attachment within your Sportsware account and also attached to this email. You may scan your physical form and email it to me, upload it on Sportsware, or fax it to (309) 556-3484.
